How PvP Works
It happens in two stages. Over level 100, open PvP is on for everyone: you can attack and be attacked anywhere outside Protection Zones, and skulls apply to unfair kills. But a kill only grants experience once you are promoted: a promoted player who kills another promoted player takes part of their experience. Unpromoted players can still fight, but their kills just give no experience.

Anyone can fight outside Protection Zones, but kills give no experience yet.

Promote
The Queen resets you to level 2; skills, magic level and items stay, plus new spells. Then climb back up.

PvP experience
Now you gain experience for killing other promoted players, and monster experience is halved, so PvP is your main way to level.
SkullsYou get a frag for killing a player who did not attack you first, unless it is a fair fight between two promoted players, or the target already had a skull
White Skull
Shown when you attack another player. While it is up, anyone may fight back and killing you earns them no frag. It fades a short while after combat ends.
Red Skull
Marks a player killer at 10 unjustified kills as a promoted player (5 if not). While red, you drop everything you carry when you die, and an Amulet of Loss will not protect it.
Black Skull
The most severe tier, reached at 20 unjustified kills promoted (15 if not). The same total item loss as red, and you also revive with almost no health or mana.
Each unjustified kill fades after 24 hours. With a red or black skull you drop everything you carry on death, and an Amulet of Loss will not protect you. Type !frags in game to check your own frags and the time until the next one wears off.
More details
Both players must be promoted. A non-promoted player gives and gains no PvP experience.
Self-defence gives full experience. If a player attacks you first, you gain full experience and no skull, at any level difference.
"Much lower" means under 67% of your level. Below that line you get an unjustified kill and almost no experience; at or above it the fight is fair.
A "big level gap" means the target is over 1.5× your level. That is where the 20-level cap and the half-on-repeat rule apply.
Summons give experience to their owner, never to themselves, and the same applies to monsters.
Below level 100 you cannot be attacked by players and lose nothing when you die.
